Description:
Provides supportive services to older adults and individuals with disabilities. Helps individuals live independently and in an environment of their choice. Offers the following services:
- Transition assistance - helps people move from hospitals, nursing homes, or other facilities back to their home.
- Options counseling - helps people make informed decisions and navigate care
- Information and referral - offers connections to services and follow-up calls
