• Requirements: - Photo identification for yourself and your spouse - Prior tax year return (or Social Security cards or ITIN document for each family member) - W-2 Form from all employment and any additional forms relating to income (including gambling winnings, jury pay, interest, and dividend statements) - All 1099 forms from other income such as unemployment, social security, and self-employment - A blank or canceled check for direct deposit option - Form 1095-A Health Insurance Marketplace Statement if applicable - If you paid child care - bring documentation listing provider name, address, tax identification number, and amount paid - If you, your spouse, or a dependent attended college during the tax year - tuition expense statement (1098T) and student loan interest statement (1098E)
