  Offers the following CalAIM Community Support Services for eligible Sonoma County adult males:

  • Housing Deposits - Offers up to $5,000 to help individuals get settled into new housing. Funds can be applied to rental deposit, last month's rent, utility deposits, furniture, and basic necessities to set up the home. Individuals must be able to prove ability to sustain ongoing rent charges.
  • Housing Transition Navigation - Offers help to find, apply for, and secure housing for those experiencing homelessness or at-risk of experiencing homelessness.
  • Recuperative Care - Provides short-term residential care for those with unstable housing who no longer require hospitalization, but still need to heal from an injury or illness. The residential care includes housing, meals, ongoing monitoring of their condition, and other services like coordination of transportation to appointments.

• Eligibility: -Must be Sonoma County adult males ages 18 years and older -Must have Medi-Cal coverage through Partnership HealthPlan (PHP) or Independent Living Systems (ILS) of California -For Housing Deposits must be able to prove ability to sustain ongoing monthly rent charges -For Housing Transition Navigation must be homeless or at-risk for homelessness -For Recuperative Care must be transitioning from hospitalization, have unstable housing, and require additional time to heal from an injury or illness
