• Description:

  Provides enhanced care management services for eligible youth and adults within Sonoma County. Provides a Community Health Worker (CHW) to coordinate all health and health-related care for the individual. This includes assistance with all aspects of physical, mental, and dental care, behavioral health, and social services. Also coordinates transportation for medical appointments. Serves the following populations of focus:

  Adults, 21 years and older:

  • Experiencing homelessness,
  • Serious mental health and/or SUD needs,
  • Transitioning from incarceration
    

  Youth, under the age of 21:

  • Enrolled in California Children's Services (CCS) or CCS WCM with additional needs beyond the CCS condition,
  • Experiencing homelessness - homeless families or unaccompanied children and youth,
  • Involved in child welfare,
  • Serious mental health and/or SUD needs (includes children with high ACEs scores),
