  Provides Enhanced Care Management, which is comprehensive care management from a single lead care manager who coordinates all health and health-related care, including physical, mental, and dental care, and social services. Facilitates entry into County services. Services are for individuals who are unhoused with behavioral health concerns.

  Serves the Following Adult Populations of Focus:

  • Experiencing Homelessness,
  • Serious Mental Health and or SUD Needs,
  • High Utilizers of Care (including emergency room/hospital visits or short-term skilled nursing facility stays in the last 6 months)
  • Transitioning From Incarceration (starting January 2026)
    • Provider enrolled as Medi-Cal Fee-for-Service (FFS): No
