Wrap-Around services are provided both at school and in the home to supports youth to remain in their family's home despite multiple challenges.

  • Eligibility: Seriously emotionally disturbed children/adolescents and adults with a serious mental disorder may apply for services when their mental illness results in a substantial reduction in personal and or/community functioning. Eligibility is determined case-by-case with consideration given to homelessness, involvement in the criminal justice system, institutionalization and/or frequent hospitalization.
  • Requirements: Mental health diagnosis by a licensed mental health provider if available, financial documentation, public assistance documentation (Documentation is needed to develop a Wellness and Recovery Plan to better meet our consumers' needs and to determine appropriate services).
