• Description: Offers case management to seniors who want to remain at home instead of living in a nursing home. A care manager makes monthly calls or visits to clients. They advocate and coordinate the following services:
  • Grab bars
  • Respite care
  • Rides to appointments
  • Emergency response support services
  • Other independent living related services
