  Offers the following CalAIM Community Support services to residents of Ventura County:

  • Respite Services: Offers temporary relief to primary caregivers and include supervision, personal care assistance, and engagement in safe, supportive activities delivered by trained staff. Service are provided on the premises of Adult Day Health Care center, weekdays during the day only.
  • Community Transition Services / Nursing Facility Transition to a Home: Assist participants transitioning from a skilled nursing facility back into a community-based home setting.
  • Nursing Facility Transition / Diversion to Assisted Living Facility Services: Supports participants transitioning from or at risk of institutional placement by facilitating entry into an Assisted Living Facility (ALF).
  • Environmental Accessibility Adaptations (Home Modifications): Provides home modification services to improve safety, accessibility, and independence.
