Description: Ensures that every senior has access to medical eye care and promotes annual, dilated eye exams. Facilitates access to eye care at no out-of-pocket expense for eligible seniors.If eligible, a comprehensive medical eye exam will be given and care for anything diagnosed at the initial exam for up to one year for the physician's services at no out-of-pocket cost.Services that are not covered: Additional services necessary for your care such as, hospitals, surgical facilities, anesthesiologists, and medications, are the patient's responsibility and beyond the scope of EyeCare America services.
Hours: Administrative Phone Hours: Monday through Friday 8 am - Noon
Eligibility: US citizen/legal resident, ages 65+, not seen by an ophthalmologist in 3 or more years, and no HMO or eye care benefits through the VA;or people who are determined to be at increased risk for glaucoma (by age, race, and family history) and have not had an eye exam in 12 months or more may be eligible to receive a free glaucoma eye exam if they are uninsured.
