Description: Nurse Home Visiting Program provides free in-home visits and support for pregnant and parenting families. Some services include: Education and teaching around healthy pregnancy and management of any complications Education and teaching related to infant care, feeding, and safety Parenting education and support Referrals to community resources.
Eligibility: Participants must:- Live in Yolo County- Be pregnant or parenting a child under the age of 2 1/2- Be low-incomeIn addition, the participant must meet AT LEAST ONE of the following criteria:- Mother or infant with a high risk or complicated medical condition such as gestational diabetes or recently discharged from the NICU- History of previous poor birth outcome, for example, prematurity, fetal or neonatal demise- Family history of child neglect or abuse (mother or baby)- Maternal mental health and/or alcohol and drug problems
