Description: Nurse Home Visiting Program provides free in-home visits and support for pregnant and parenting families. Some services include: Education and teaching around healthy pregnancy and management of any complications Education and teaching related to infant care, feeding, and safety Parenting education and support Referrals to community resources.Home visiting referral form
Eligibility: Participants must:- Live in Yolo County- Be pregnant or parenting a child under the age of 2 1/2- Be low-incomeIn addition to the above criteria, participants may meet the following criteria:- Birthing parent or infant with a high-risk or complicated medical condition such as gestational diabetes or recently discharged from the NICU - History of previous poor birth outcome, for example, prematurity, fetal or neonatal demise- Family history of child neglect or abuse (birthing parent or baby)- Birthing parent mental health and/or alcohol and drug problems
