Description: Community Health Workers (CHWs) offer extra care management services to Medi-Cal members at no cost for those who have complex needs and challenges that make it difficult to improve their health. Works with clients to help them get the health care and social services needed by connecting individuals to various community and social service resources.CHW services can help Members receive appropriate services related to:Perinatal CarePreventive CareSexual and Reproductive HealthEnvironmental and Climate-sensitive Health IssuesOral HealthAgingInjuryDomestic violence and other violence prevention services

Eligibility: Medi-Cal members who have complex needs and challenges.
Requirements: Proof of Medi-Cal Number and/or Health Plan Member Number.
