Description: Provides physical modifications to a home environment that are necessary to ensure the health, welfare, and safety of individuals with asthma and complex, physical, behavioral, developmental, and social needs. Enables the individual to function in the home without acute asthma episodes.Examples of environmental asthma trigger remediations could include:Allergen-impermeable mattress and pillow dustcovers;High-efficiency particulate air (HEPA) filtered vacuums;Integrated Pest Management (IPM) services;De-humidifiers;Air filters;Other moisture-controlling interventions;Minor mold removal and remediation services;Ventilation improvements;Asthma-friendly cleaning products and supplies;Other interventions identified to be medically appropriate and cost effective.The program is supported by CalAIM, also known as the Medi-Cal Transformation Initiatives. Hours: Health Care Options: Monday through Friday, 8 am - 5 pm
Eligibility: Asthmatic idividuals who have Medi-Cal and are enrolled in a Managed Care Plan with complex physical behavioral, developmental, and social needs.Currently, CHC serves members eligible for ECM with Medi-Cal Managed Care through:- Anthem Blue Cross- Health Plan of San Joaquin- Mountain Valley Health Plan- Partnership Health Plan- Central California Alliance for Health
