Description: The Assisted Outpatient Treatment (AOT) FSP program serves individuals 18 years of age or older, diagnosed with chronic, serious mental illness or co-occurring mental illness and substance use disorders, who have been clinically determined to be unlikely to survive safely in the community without supervision, who have a history of non-compliance with outpatient mental health treatment, and whose condition is substantially deteriorating and likely to result in grave disability or serious harm to self or others. Core Services include Benefits assistance, Case management, Client self-help and peer support services, Crisis intervention 24 hours/day, 7 days/week, Education, support and housing assistance, Evidence-based mental health treatment, Linkage to physical health care, Medication support, Substance use services, and Vocational services.
