Requirements: Only collected as needed for programs. Not all will ask for proof of any kind. Latest Federal Tax Return, W-2, pay stubs (2 months), Social Security Award Letter, Unemployment Benefits Letter / Insurance Cards / List of current medications or actual prescriptions-used to conduct assessment for programs
