Description: Provides wraparound services which include planning process and a comprehensive range of supports and services to children and families who have complex needs. Services can be offered in home and/or community settings, and can include the following: team meetings, creating action plans, case management/linkage, crisis intervention, collateral services, plan development, flex funds, peer support, mental health assessments, and evaluations. Serves Families in East Contra Costa County.
