Includes the following services:Medical and healthcare case planningHealth and Education Passport (HEP)Helps foster caregivers get comprehensive health assessments and dental examsExpedites referrals for medical, dental, mental health, and developmental servicesCoordinates health services for children in out-of-county and out-of-state placementsProvides medical education through the interpretation of medical reportsTraining for foster team members. Trains on the special health care needs of children and youth in foster care.Services offered in consultation and collaboration with social workers and probation officers.
