Description: Offers an evidence-based program with demonstrated effectiveness in reducing symptoms and increasing the quality of life amongst older adults who are experiencing symptoms of isolation, depression and /or chronic health conditions. Services include: Training for problem-solving skill-building Goal setting skill building Skill building coaching Social and physical activation Progress review Referrals to appropriate services Follow-up services
