Eligibility: Serves low-income seniors and veterans. Participants must meet the following criteria:. Be at least 55 years old;. Must live in Sacramento County;. Must be eligible for nursing home level of care as determined by the State of California (must have a chronic condition that needs to be managed);. Must live at home or in a community setting without jeopardizing their health or safety.
