Description: Provides services in a home-like environment (up to a 30-day stay) to help youth transition back into community living after a period of psychiatric crisis and recovery. Residents receive short-term counseling and services that support their transition to housing, employment, education, and ongoing care. Services: Provides mental health services such as individual counseling, group counseling, and psychiatry.Incorporates the tenets of recovery, peer support, and identifying community connections.Assists youth in creating their own Wellness Recovery Action Plans.Provides culturally competent services for LGBTQIA+ youth. Hours: Crisis Facility and Intake: Seven days a week, 24-hours;Office Hours: Monday through Friday, 9:30 am - 5:30 pm
Eligibility: Transition-age youth and young adults ages 18 - 29 who are looking to successfully transition back to community living after a period of psychiatric crisis and recovery.
Requirements: Crisis Residential Program Referral Form and LIC602 Form. Other supporting documentation on a case-by-case basis.
