Description: Provides youth short-term counseling and services in a home-like environment (up to 30 days stay) to help youth transition back into community living after a period of psychiatric crisis and recovery. Residents receive counseling and services that support their transition to housing, employment, education, and ongoing care. Services Includes: Provides mental health services to include individual and group counseling, as well as psychiatry Connects residents to housing, education, vocational, healthcare, and other community-based services Incorporates the tenets of recovery, peer support, and identifying community connections Assists youth in creating their own Wellness Recovery Action Plans. Hours: Crisis Facility and Intake: Seven days a week, 24-hours;Office: Monday through Friday 9:30 am - 5:30 pm
Eligibility: Transition-age youth ages 18 - 29 looking to successfully transition back to community living after a period of psychiatric crisis and recovery.
Requirements: A referring agency must fill out a Crisis Residential Program Referral Form and a LIC602 Form on behalf of the patient. Other supporting documentation should be included on a case-by-case basis.
