Description: Provides comprehensive, psychosocial rehabilitation and recovery for voluntary adults, with severe mental illness, co-occurring and dual diagnosis. The program creates an environment that promotes recovery, with a schedule of daily groups and bi-weekly individual sessions to assist clients in developing independent living skills to enable clients to successfully step down to a more independent level of care.
Clients are welcomed into a warm, homelike atmosphere where emphasis is placed on identifying individual motivating factors and goals toward recovery.
Services also include case management for the Public Conservator's Office, medication support with nursing services 12 hours/day 7 days/week, psychiatry on site once a month and telehealth every Friday, vocational services, peer support, and milieu therapy with staffed mental health professionals onsite 24/7.
Hours: Office Hours: 8:00 am-5:00 pm, Monday-Friday; Facility Hours: 24 hours a day, 7 days a week
Eligibility: Ages 18 and older. Must have qualifying Title 9 mental health diagnosis with a history of unsuccessful attempts at maintaining stability with a lower level of care in the community.
Clients must have a willingness to participate in program and SUD outpatient treatment if history of substance abuse. Clients with non-psychiatric medical needs cannot require more than outpatient care. Clients cannot be a current imminent danger to self or others.
