Description: Provides in-home services to patients and families facing terminal illnesses. Team consists of physicians, nurses, home health aides, clergy or other counselors, trained volunteers, speech, physical, and occupational therapists.Service includes: Pain and Symptom Management Counseling and Supportive Services Spiritual Counseling Therapy Services Personal Care and Home Health Aide Services Volunteer Services Supplies, Medication, and Durable Medical Equipment Related to the Life-Limiting illness Continuous Care Respite Care Services Inpatient Services
Hours: Clinical on-call: 24 hours a day, 7 days a week, 365 days a year
Eligibility: Must be 18 years of age, diagnosed with advanced chronic and/or life limiting illness and under a physician care, and have less than 6 months of living.
