Description: Provides a center where medical and social services are provided to help individuals stay living at home, delay getting institutionalized, and to minimum hospital visits. Activities in lunch room, a computer lab, an exercise/physical therapy room, and a courtyard are available for use. Respite care is also available for the caregivers.Services include: Occupational therapy Physical and speech therapy Nursing services Psychological and social services Medication management Hot meal and nutritional counseling Door-to-door transportation
Eligibility: Must meet all criteria:*Must be 18 years of age or older*Have a chronic medical, mental or cognitive condition including arthritis, diabetes, past stroke, dementia, Alzheimer's, high blood pressure, and depression*Must have Medi-Cal Managed Care Plan such as Health Net, Health Plan of San Joaquin, Straight Medi-Cal or Kaiser.
