Description: Provides end-of-life services to patients living at home or in a facility which serves as home. Services include nurses, physicians, aides, social workers, and other members of the team. Program includes coverage for all medication and equipment associated with a terminal diagnosis. Bereavement counseling available in groups or individually as needed.
